One Time Consultation

Not sure whether your loved one is ready for FamilyBridge? Our One-Time Consultation is an optional service designed for families who want expert guidance before deciding if ongoing care management is the right next step.

During this comprehensive consultation, we take the time to learn about your loved one's health, living situation, support system, daily routines, and current challenges. Together, we identify immediate concerns, discuss available care options, and develop personalized recommendations tailored to your family's goals and circumstances.

Following the consultation, you'll receive a clear action plan outlining practical next steps, helpful resources, and recommendations designed to help you make informed decisions with confidence. Whether your loved one would benefit from one of FamilyBridge's ongoing service plans or your family chooses to move forward independently, you'll leave with greater clarity and a personalized roadmap for the future.

This consultation is ideal for families who are just beginning their caregiving journey, experiencing a recent change in a loved one's health or independence, or simply looking for an experienced professional to help determine whether additional support is needed. There is no obligation to enroll in any FamilyBridge service after your consultation, our goal is simply to help you make the decision that is best for your family.

FamilyBridge Essentials
$350.00 every month

We start with our intro meeting, where we begin with this form

Then we will schedule once-a-month home visits where we will check all of these items every visit:

  • Refrigerators

  • Stovetops

  • Ovens

  • Inside trashcans

  • Outside trashcans

  • Evidence of rodents and insects

  • HVAC

  • Carpets

  • Flooring

  • Rugs

  • Laundry system

  • Bedroom/ bed

  • Bathrooms (toilets and showers/tubs)

Both the care recipient and emergency contact will receive the PDF document of our visit note, where we lay out our findings and recommendations.

FamilyBridge Essentials is designed for older adults who are living independently but would benefit from professional oversight and periodic support. Through monthly check-ins, personalized guidance for adult children, and ongoing communication, we help identify concerns early, connect you with trusted resources, and provide peace of mind knowing someone is looking out for your loved one's well-being. It's an ideal solution for families seeking reassurance and expert guidance as they navigate the aging journey while still allowing Mom and Dad their independence.

Family Bridge Active Support
$625.00 every month

Then we will schedule twice-a-month home visits where we will check all of these items:

  • Laundry system

  • Bedroom/ bed

  • Bathrooms (toilets and showers/tubs)

  • Airfilter

  • Smoke detectors

FamilyBridge Active Care offers a more hands-on level of support for families whose loved ones are beginning to experience changing health needs or require more frequent oversight. We provide proactive care coordination, twice-monthly home visits, aid in coordinating healthcare professionals, and ongoing advocacy to help prevent small concerns from becoming larger challenges. By serving as a trusted partner for both seniors and their families, we help simplify complex situations while providing confidence, a friendly face, continuity, and peace of mind.

FamilyBridge Concierge
$1,100.00 every month

Then we will schedule once-a-week home visits where we will check all of these items:

  • Outside lawn maintenance (back and front)

We also provide a recommended list of service providers in the area just for our concierge members (ex., cleaning service, lawn care, handymen, meal coordination), and will help book and coordinate these needed appointments.

What Plan Is Right for Me?

Every family's situation is different, which is why FamilyBridge offers multiple levels of service. While each package is built on the same commitment to compassionate guidance and expert advocacy, they differ in the level of involvement and ongoing support we provide.

  • Start here. You're trying to understand the situation and determine what kind of support may help.

  • Regular reassurance. Mom or Dad is largely independent, but you'd like a regular local presence, family updates, and help with practical details.

  • More support and coordination. Your family needs more frequent involvement, communication, organization, and practical follow-through.

  • Our highest level of support. You want frequent visits, priority communication, and more extensive help coordinating the practical details of aging in place.
